Many Filipinos have this uncomfortable experience of seeing typographical errors, wrong spellings and even discrepancy in their Philippine birth certificate and such typos make it more difficult to apply for legal documents particularly passports and visas. So what will you do when you see such mistakes in your NSO birth certificate? Where to File Petition to Change Name in Birth Certificate
Inquire from the municipal or city civil registrar’s office where you were born. if there are typographical errors in the spelling of your name in your birth certificate, you canĀ ask for the necessary correction from that particular office.
Requirements
You will also need several documents when filing for your petition. Supporting documents are required to correct the clerical error in your birth certificate. Make sure to prepare at least two documents that will be used as basis for the correction. Baptismal certificate, employment record, voter’s affidavit, NBI/police clearance, civil registry records of ascendants and school records are generally accepted.
Fees
To correct typographical errors in Philippine issued birth certificates, a fee of P1,000.00 is collected.

Really it’s only 1000, but someone from the city hall offices was asking for 3000 pesos.
I have a typo error on my surname, is my father’s birth certficate cannot be used because it is a late registered?….do the documents i need to submit should be older than me…
